Confirm that Avid/Premiere is not outputting in 5.1 (must be 2 channel stereo).
Click the gear icon under the audio meter that is being used and go to properties. Confirm that the device is selected as an audio source.
Unplug the HDMI from the capture device for 3 seconds, plug back in.
Restart the Avid workstation.
